Reserve Form for Faculty
NOTE:
This form is for book
and similar materials only. The library is not accepting photocopies as hard copy reserves any longer. All
photocopy reserve materials are now to be placed on Electronic Reserves
(ERes) only (see the Library's Information Page on the Electronic
Reserves site, Lib001, for downloadable booklets on ERes and how to use
it).
To
use this Reserve Form, simply fill out completely each field with the
appropriate information, then click the Send button. Or, if you
prefer, you may fill out the Instructor, Course Number/Name, and
Semester/Program areas only, then print it out for attaching to your
syllabus and sending via campus mail to the library.
Material
to Be Reserved
Please
provide as complete citations as possible. Incomplete information
leads to complications for students. Instead of enumerating
materials below, you may after filling in the information above, print
out this form and attach it to a copy of your syllabus. In any
case, whichever way you choose to do it, this form must be submitted
before your materials can be processed.

NOTES
1. All
personal material will be returned to you within 2 weeks of the
last date of the semester for which the material was reserved.
2. There is
a restriction of 15 books per course during one semester.
3. The
library complies with Title 17 of the U.S. Code concerning
Copyright Law. See the Library and Faculty Guide to
Copyright posted on the Library's web site (About the
Library|Policies|Copyright). The Library at Spalding
University considers the Library Reserve Room as an extension of
the classroom and interprets and accordingly applies the Fair
Use Doctrine to its materials and their use.
Please
allow 3 working Days for materials being placed on Reserve
